to make an appointment to take a school tour. We highly recommend you to take a school tour prior to submitting an application; however, it is not required. |

| 2. Visit the School: 4511 Peralta
Blvd, Fremont, CA 94536 |
During your visit, you will be introduced
to the Montessori educational philosophy,
go on a tour of the school, and also be
able to observe in a classroom. You may
bring your child with you. |

| 3. Meeting with the Director |
After your observation, the director will
meet with you and answer any questions you
may have and an application form and a
brochure will be given to you. You may
also download the Admissions Application and bring it with you. |

| 4. Submit Application /Application Fee: |
A $100.00 non-refundable, non-deductible
application fee is due with the
application in order to confirm and hold
your child's place.

| 5. Acceptance and Enrollment Packet &
Fees: |
Upon receipt of the application and the
$100.00 non-refundable, non-deductible
application fee, an enrollment packet will
be given. The parents will complete, sign and return all portions of the
enrollment agreements, emergency forms, health history forms, immunization records and all other required forms on child's first day of school. For your convenience these forms are listed below and can be downloaded from the links below. |

| 6. Child's First Day of School Checklist: |
On child's first day of school, make sure to bring the following (forms must be complete and signed)
1. Admissions Application, with Registration Fee if not already submitted.
2. First month's tution.
3. PERSONAL RIGHTS- FORM LIC613A.
4. CONSENT FOR EMERGENCY MEDICAL TREATMENT- FORM LIC627.
5. IDENTIFICATION AND EMERGENCY INFORMATION- FORM LIC700.
6. PHYSICIAN’S REPORT- FORM LIC701.
7. CHILD’S PREADMISSION HEALTH HISTORY—PARENT’S REPORT- FORM LIC702.
8. NOTIFICATION OF PARENTS’ RIGHTS- FORM LIC995.
9. PARENT HANDBOOK AGREEMENT.
10. CAREGIVER BACKGROUND CHECK PROCESS.
11. Copy of updated CALIFORNIA SCHOOL IMMUNIZATION RECORD. (PM 286)
12. Two sets of extra clothing
13. Crib size bed sheet and blanket (if your child takes a nap)
14. Pull-ups and wipes (if your child is not potty trained)

| Waiting List: |
If there are no spaces available in the
program to accommodate your child, you may
choose to be placed on the waiting list.
There are no fees to place your child on
the waiting list. |
